Please pick it up beforehand or fill it out on the day of your vaccination.<\/li>\n\n\n\n<li>If you receive a subsidy from a municipality other than Kutchan, you must present a vaccination request form issued by your municipality. Please contact your municipality in advance before making an appointment.<\/li>\n\n\n\n<li>For companies or organizations wishing to make a group payment, a dedicated reservation menu will be created. Please consult our clinic in advance.<\/li>\n\n\n\n<li>For groups of 10 or more people, we also offer on-site vaccinations at your company or organization.
